Features it is intense - deformed states the head of the humeral bone at various variants of the loading

Now there are two basic methods of treatment of depressed fractures of the head of a humeral bone.

  • First - торсионная an osteotomy of the proximal extremity of a humeral bone with the purpose of removing the defective field of the head from zone the fissile loading.
  • Second - filling of defect by various stuffs with the purpose of regeneration of the anatomical form of the head of a humeral bone and building of a high-grade leg in zone loadings. As a filler various stuffs such as auto-and allobone, various views of ceramics arc used. However, despite of all positive moments of numbered procedures, results of treatment not always satisfy both doctors, and patients.

The purpose of the given biomechanical examination has consisten in the following, by mathematical model operation to reveal the causes adducting in negative takes, and to spot an optimum method of treatment of the given pathology.

For carrying out of examinations with use of a method of terminating devices by us the model of a proximal department of a humeral bone which includes the following devices has been built: the humeral bone consisting of two layers (the periblast has performances кортикальной bones, intrinsic - performances of a spongiform bone), a cartilage and an articulate surface of a scapula (glenoid). Requirements of a loading of a shoulder joint in norm were simulated and at presence additional вывихивающей forces.

The received data, at examination of a shoulder joint in norm, show, that the peak strain in a shoulder joint arise in glenoid and achieve 162,7 МПа, and in the head of a humeral bone there are strains in кортикальной bones and achieve 37,5 МПа.

Analysis of the received data, at presence of additional choronomic (biasing) effort, shows, that at identical вывихивающей to force of a strain in the head of a humeral bone, in case of a forward dislocation it is much more (42,0 МПа), than at a posterior dislocation (37,0 МПа) and on 10-12 % it is more, than in variant without additional effort. It is necessary to note also, that the peak value of strains in range of a joint has changed, which is in glenoid, in the first case has considerably increased and has achieved 336,3 МПа, and in second - has a little decreased (101,6 МПа).
