Application of "ao" plates in surgical treatment of diaphysial fractures of the long bones of extremities

One of the most widespread modern methods of surgical treatment of diaphysial fractures of the long bones of extremities is extraosseous osteosynthesis by "AO" plates.

The analysis of results of treatment of 156 patients with diaphysial fractures of the long bones of extremities, who stayed in the orthopaedic department of the Lugansk regional clinical hospital in 2003-2005, undergone the surgical treatment with application of extraosseous osteosynthesis by "AO" plates, was taken as a principle in this article. The victims had fractures due to direct injuries in the traffic accidents - in 68,6% of cases, as well as, traumas of everyday life (22,4%), and occupational traumas (14,0%). Male patients (82,1%) of the able-bodied age prevailed. 25 patients with fractures of diaphysis of humerus, 4 - with ulna, 13 - with radius, 31 - with both humoral bones, 26 - with femoral, 18 - with tibia, 39 - with both bones of a shin were operated. In the middle one third of diaphysis the fractures took place in 69,9% of cases, in the upper one third of diaphysis - in 23,1 %, and in the lower one third of diaphysis - in 7,0% of cases. More than 2/3 of fractures were of the splinter type В and C. In 36 cases there were open fractures.

In the urgent order 27,6% of patients were operated, additionally - 23,7% of patients in urgent-postponed order, the rest patients were operated during one month after getting a trauma. Every operation had always been planned in such a way to perform it atraumatically, observing the regulations of the classical operative technique of application of "AO" plates.

In the postoperative period, when the patients had fractures of the splinter type, in separate cases, additionally, the plaster-of-Paris bandage was used for 1-1,5 month period. The aspect concerning the terms of loading on the operated extremity was always solved in dependence of the morphology of a fracture.

The results of treatment in the terms up to 1 year were observed in 128 (82,1%) patients. In 114 patients the knitting consolidation of a fracture was achieved and the results were evaluated as good, in 8 cases the repeated operative intervention with additional osteoautoplasty was in need, in 4 cases, during treatment of open fractures, and in 2 cases, during treatment of close fractures, traumatological osteomyelitis occurred.
