Treatment of oblique and spiral diaphisis crus bone fractures with elastic bended kirshner's wires in the system of constant skeletal traction

Fast development of orthopedic surgery widened indications for surgical treatment of fractures, their complications and numerous pathological conditions. But the value of skeletal traction in many cases remains irreplaceable.

Aiming increase of treatment's efficacy in patients with open and closed diaphisis crus bone fractures with oblique and spiral configuration we have developed a complex of special curing methods. They included: closed reposition of bone fragments with Kushner's wires in the system of skeletal traction, coordinate grid, directing device for intrusion of wires into bones, special damper suspensions, modificated frame of Boler's tire, device for placing restricting elements in wires both during penetration of bones and their removal.

Introduction into clinical practice of suggested methods of side compressing osteosynthesis with wires was grounded with theoretical and experimental investigations held on 24 isolated cadavers crus bones. As a result it was found that wires should be inserted into bone either through one wall (author's certificate of the USSR # 976962) or both walls (author's certificate of the USSR # 1248597) under the angle of 50-60 degrees relatively the axis of crus, outside fracture zone. In such a situation bended wires provide proper immobilization of bone fragments.

Usage of suggested complex in 147 patients with open and closed, oblique and spiral diaphysis crus bone fractures, not complicated and complicated with damage of skin (wounds, penetrations, bubbles) in the zone of fracture created favorable conditions for primary healing of fractures, reduced duration of treatment and increased its efficacy.
